OUTCOMES
The need for significant investment in the primary and community care infrastructure to ensure that the physical capacity required to deliver the proposed service model and associated changes in the patterns of delivery, can be facilitated
As a consequence of this investment, and with the full engagement of staff, there is an expectation that a range of outcomes will be achieved including:

improved management of patients’ conditions
Development of local clinical networks to enable primary care led management of patients
Focus on self-care leading to improvements in health and wellbeing
Provision of a range of public health and social care programmes and greater integration between those programmes
Development of direct booking from primary care
The provision of diagnostic services (where appropriate) to support the clinical management of patients in primary care
The establishment of new integrated care pathways for the management of chronic diseases
A greater percentage of patients managed in primary and community care settings
Greater access to health and social care services in community settings
Provision of community rehabilitation to support patients in the community and earlier discharge from hospital
Consultant outreach/engagement to support the management of patients in primary and community care settings
A reduction in the number of hospital admissions for patients with chronic conditions
Reduced length of stay in acute settings
